- 申請更換指導教授前，應先與原指導教授充分溝通並保留紀錄。
- 更換指導教授後，學生須同時更換研究主題並繳回原研究紀錄。
- 本表經各方簽章後，送學程主任核准生效。

- Prior to requesting a change of advisor, the student must communicate fully with the current advisor and keep a record.
- Upon changing an advisor, the student must also change the research topic and return all original research records.
- This form becomes effective after signatures from all parties and approval by the program director.
